Providing excellent wound care services depends on documentation that clearly illustrates assessment and medical necessity of the client, skilled interventions performed, and outcomes achieved. Complete and thorough documentation is the critical foundation for successful reimbursement of therapy wound care services. When it comes to regulatory audits and denials of reimbursement based on documentation, being proactive is the key. This session will address pertinent topics such as components of resident centered collaboration between therapy and nursing in wound care, common errors and key elements in wound care documentation, and principles of effective documentation for nursing and therapy to support skilled wound care services.
